no callerID from ITSP Brekeke sends back 486 busy here

windows 2K3 Sp1
4. UA (phone), gateway or other hardware/software involved:
ITSP
5. Your problem:
When we receive calls with no callerID from ITSP Brekeke PBX sends back a 486 busy here

Put a dummy From-URI (as Caller-ID) with the DialPlan.

And it still is begin rejected with 486 busy

It should be in the Deploy Pattern...
Remove "From=" definition from the Matching Patterns.

Also I recommend you to use the latest version (3.5) because it has DialPlan History feature which helps your dialplan debugging.
