It worked
It worked. The original problem turned out to be ATA I was using to call the number. That ATA was in the same local network as the Asterisk, with the Brekeke SIP server out on the public domain. Not sure what you mean
You can only do SIP to SIP.
If you mean between two UAs instead of to an ITSP, yes you can.

Alternate solution
Are you trying to repacketize the RTP data using ptime ?
One solution I have working is OSS with custom version of RTPProxy which repacketizes G.723 and G.729 payloads in 30ms, 60ms and 90ms payloads to save huge on bandwidth.
Is this what you are trying to do with ptime ?
