REFER sends call back to originator

I have 2 devices registered with proxy. I am trying to transfer a call, but it's not working. All calls route proxy --> Gateway --> PSTN. Here is my scenario

A calls B (A and B are registered with proxy)
B answers
A calls C (C is not registered. It's a PSTN phone)
C answers
A transfers B to C
I see the REFER arrive at the proxy, but it's turned around and sent back to my PBX. It should be forwarded to my voice gateway, but it's not.

I wouldn't think I need a special dial plan for this, but maybe I do.

Any thoughts?

Which PBX product and SIP client are you using???
